Step 1:

  • Download, print and fill out the initial applicant face sheet. All fields are required.


  • Download, print and use checklist to ensure all the required documents are submitted.

Step 2:

  • Download, print and fill out all application packet pages. (I-9 Form and W-9 Form) All fields are required.


Note:

  • Notarized and sign "affidavit of good moral character and independent contractor waiver."

  • Enclose 2 reference check sheets with your previous or current employer's name, address, and phone number.

Step 3:

When your required documents are completed, call (561) 304-1311 to make an appointment so that one of our staff members can review your paperwork.
